I was on my way to exit the Safeway store when I had this bad fall. There was no sign at all that the floor was slippery. To my embarrassment, I struggled to lift myself up. There was this nice man , Tony Brown, who asked if I was alright and that I should tell the staff. That is what I did then.

I did then talk to the Manager and asked me if I wanted to file a complaint. I said yes and so he gave me the form. I gave details then of the incident.

It was around 5:15 PM, 12/27/2009 when the accident happened. Right now it is 7:15 and I am aching all over. My ankles, bottom back, wrists, elbows and knees are hurting.
